Services
LDS’s services support at risk youth by helping them learn and practice the skills necessary to successfully transition into adulthood.
Services include but not limited to apartment living services, supportive case management, anger management counseling, life skills training development, employment services, educational referral services, mentorship, medical and mental health services.

Services Provided:
Life Discovery Services support at risk youth by helping them learn and practice the skills necessary to successfully transition into adulthood. Services include but not limited to apartment living services, supportive case management, anger management
counseling, life skills training development, employment services, educational referral services, mentorship, medical and mental
health services.
Our life skills training development services are provided based
on the Casey Life Skills Assessment and the development of an Individual Service Plan (ISP) at the time of admission to the
program.
Life Coaches will be assigned to youth to provide the following services:
• Case Management Services;
• Life Skills Development/Training;
• Budgeting/Financial Management
• Educational Supportive Services Referrals;
• Employment Services/Job Readiness Training;
• Transportation Assistance;
• Mentoring Services/Connection to Community Resources;
• Anger Management Counseling; and
• Referral to Medical and Mental Health Services.
